Paciente de edad avanzada con cáncer colorrectal: toma de decisiones

La mayoría de los casos de cáncer colorrectal (CCR) se diagnostican en personas mayores de 65 años, un grupo marcado por alta heterogeneidad clínica y funcional. La valoración geriátrica integral (VGI) se ha consolidado como una herramienta esencial para personalizar y adaptar el manejo terapéutico al perfil de fragilidad de cada paciente. En el contexto del CCR, la elección de la estrategia terapéutica requiere equilibrar el riesgo de mortalidad atribuible al tumor con el beneficio potencial en supervivencia derivado del tratamiento. En pacientes de edad avanzada, esta decisión debe considerar de manera prioritaria el potencial mayor riesgo de toxicidades, la esperanza de vida independiente del cáncer y las preferencias, valores y objetivos del paciente. En este artículo revisaremos el concepto de fragilidad, las diferentes herramientas para su evaluación, y el impacto que su determinación tiene en las diferentes opciones de tratamiento del CCR.
